The Packet Cover Sheet (MFL-111) is a cover page that organizes and identifies all the documents being submitted together in a court filing, helping the court clerk sort and process your paperwork correctly. People filing their own cases without a lawyer—such as those going through divorce, small claims, or family law matters—would need this form to bundle their documents properly when submitting them to the court.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
