This form allows a person subject to a juvenile court firearm prohibition order to request permission to possess a gun or ammunition specifically for their job or work duties. A person under a firearm restraining order or firearm prohibition in juvenile court uses this form to ask the judge if they can have a gun or ammunition at work, even though the court order normally bans them from having firearms.
